A list price is the published price from a drug maker.
Read note A| Part of the quote | Question to put beside it |
|---|---|
| Assessment | Is this only the opening visit, or does it include follow-up? |
| Medicine or skin product | What exact item and supply period does the amount cover? |
| Tests | Which tests are proposed, and who bills them? |
| Membership | What does it include, and what is charged separately? |
| Delivery | Is sending the item included? |
| Renewal | What changes after the opening period? |

Leave missing amounts open
